You can't get bi's and tri's like that without getting huge shoulders and forearms. And he has neither.

Yeah, looks like it could be easily Photoshopped.
He's real, but this is caused from synthol injections or similar. Not a steroid but an oil. Some pro-bodybuilders use it cosmetically to enhance lagging body parts. And some just do stupid things with it like, well, that guy.
